Our team has a very unique opportunity where social workers manage a small cohort of patients while having access to ample support resources, all intended to ensure employee success and patient safety. The Comprehensive Bleeding Disorders Clinic at Emory Midtown is open once per week where social workers connect directly with our patients and team. The remainder of the week consists of working in-office or remote based on employee preference and team needs. Our social work team collaborates with providers and members of the team to triage, coordinate, and consistently manage patient care. Social workers assume the role of a health care liaison between internal providers and community providers. Our social workers are essential to improving outcomes for our patients and critical to the overall success of our center.
