Provides emotional support and psychosocial counseling to patients. Assists in a multi-disciplinary approach to patient care. May identify discharge needs and arrange for comprehensive services for patients following discharge from the hospital. Performs related responsibilities as required.
Bachelors degree in social work, psychology or a related field. Two years of social work/social services experience. Positions within this classification may require licensure as a licensed professional counselor (LPC) or licensed social worker.
As a member of a multi-disciplinary day program, research programs, and/or cognitive neurology clinic the individual in this position will be focused on patients and families affected by mild cognitive impairment and dementias. The individual’s primary responsibility will be providing services and support to people living with MCI or dementia and their families.
Services provided will include personalized, problem-oriented and detailed patient assessments, goal-setting, emotional support, brief supportive therapy, psychoeducational group work, case management, and transition planning. Participate in a multidisciplinary team that develops treatment plans, provides patient and family education and serves as a professional resource. Provides individual, family and group therapies.
